General medicine case-7


 This is an online E log book to discuss our patient's de-identified health data shared after taking his/her/guardian's signed informed consent. Here, we discuss our individual patient's problems through series of inputs from available global online community of experts with an aim to solve those patient's clinical problems with collective current best evidence based inputs. 

CHIEF COMPLAINTS:-

A 60 yrs old male,  goat farming by the occupation presented to the opd with the cheif complaints of Distended abdomen since 20-30 days, pain in the abdomen since 1 week, decreased urine output since 2days and constipation since 2 days.

HISTORY OF PRESENT ILLNESS:- 

Patient was apparently asymptomatic 1 month back  then he developed abdominal distension since 20-30 days  which is progressive in nature and is associated with pain since 1 week with decreased urine output and constipation since 2days. 

Patients daily routine is, he wakes up at early in the morning 5:30. He will have his food at 9'0 clock.He goes to his work at morning 11'o clock. He takes dinner at 8'o clock and sleep by 10'o clock. 

PAST HISTORY:- 

There is no history of hypertension, diabetes mellitus , tuberculosis, asthma, and epilepsy

There is no history of any surgery.   

PERSONAL HISTORY:-

Married

Patient takes mixed diet.

Sleep- increased pattern of sleeping

Appetite- normal

Bowel moments are regular.

Occasionally drinks alcohol.

Gutka chewer since 7months.

GENERAL EXAMINATION:- 

Patient is coherent and cooperative. 

He is well oriented to time , place and person . Moderately built and nourished.

No anemia 

No Pallor 

No icterus

No cyanosis

No clubbing

No generalized lymphadenopathy 

VITALS

TEMP- Afebrile

PR- 100 bpm

BP- 120/80 mmHg

RR- 16 cpm

GRBS- 99 mg/dl 

SYSTEMIC EXAMINATION:- 

ABDOMINAL EXAMINATION:

SHAPE- DISTENDED

TENDERNESS- Present

FREE FLUID -present

No palpable mass

No bruit

Liver and spleen are not palpable

Bowel sounds present.

ABDOMINAL GIRTH 87 cm

CVS: S1S2 + NO MURMURS

RS: BAE+ NVBS

CNS: NFD+
