Here, we discuss our individual patient's problems through series of inputs from available global online community of experts with an aim to solve those patient's clinical problems with collective current best evidence based inputs.  CHIEF COMPLAINTS:- A 60 yrs old male,  goat farming by the occupation presented to the opd with the cheif complaints of Distended abdomen since 20-30 days, pain in the abdomen since 1 week, decreased urine output since 2days and constipation since 2 days. CHIEF COMPLAINTS:- 22 years old female daily wage labourer by occupation presented to the opd with c/o pain and swelling in left lower limb since 1 month . HOPI Patient was apparantly asymptomatic 1year ago then she had  round skin lesions which are associated with itching then they went to local doctor and used some topical ointment for period of 3-4 months patient did not got any relief so they again went to doctor then patient was prescribed with avil and decadran injections and patient use
